HEDIS® is one of the most widely used sets of health care performance measures in the U.S. Whether you’re new to health care or a seasoned professional navigating quality measurement, digital transformation or regulatory changes, understanding HEDIS is essential. That’s why NCQA recently updated HEDIS 101—a foundational course that traces the origins of HEDIS, explains how it works today and previews where it’s headed.

HEDIS Began in Partnership With Large Employers

HEDIS originated from conversations between large employers and health plans about the need to improve health care quality. It wasn’t long before they reached consensus: The nation needed a standard way to measure quality that would enable health care purchasers to compare health plans. Their solution was the Health Plan Employer Data and Information Set, later renamed “Healthcare Effectiveness Data and Information Set.”

NCQA was a new organization when it became responsible for developing and maintaining HEDIS. The first HEDIS measure set was released in 1993, and quickly became the national standard for health plan performance reporting.

HEDIS Defines Quality and Value in Health Care

HEDIS helps purchasers make “apples-to-apples” comparison of health care organizations by answering questions such as:

  • How effectively are care and services delivered?
  • How easy is it to access care?
  • Are evidence-based health care services provided to the right person at the right time?
  • Do services lead to better health outcomes?

An important—and unique—component of HEDIS is its attention to statistical details. HEDIS measures use a set of rules that can be unambiguously interpreted and consistently applied across organizations. Every organization that reports HEDIS must undergo a HEDIS Compliance Audit™—an independent, objective review of its information systems capabilities and HEDIS reporting processes to certify the accuracy of results.

Because they are a trusted source of quality data, HEDIS measures are included in value-based arrangements between health plans and providers, as well as in government quality performance programs such as the Centers for Medicare & Medicaid Services Medicare Advantage Star Ratings Program and Medicaid Core Sets. NCQA also uses HEDIS data as part of its Health Plan Ratings and makes that data available to help benchmark plan performance through its Quality Compass® tool.

The Future of HEDIS Is Digital

HEDIS measures are evolving to be “digital first.” HEDIS digital quality measures are an important part of NCQA’s strategy to improve quality measurement and encourage the exchange of health information. In 2021, NCQA began offering HEDIS digital measures that use the Fast Healthcare Interoperability Resources (FHIR®) standard, which enables better exchange of electronic health information, and aligns with new regulations that support interoperability. NCQA’s roadmap plans to make HEDIS fully digital by 2030.

Almost 400 quality advocates from 30 states gathered in Los Angeles March 4-5 for NCQA’s first Health Equity Forum. Day 1 focused on the big-picture “why’s and how’s“ of improving health equity, including the role of public policy. Day 2 was an interactive dive into NCQA Health Equity Accreditation. This successful event is a model for future NCQA conferences about building better, more equitable care.

Big takeaways from LA include:                                                                                                                

  1. Embrace collective action. Health equity cannot be achieved by individual actors alone. The work is hard and complex, and demands collaboration across sectors. Enlisting allies also means you’ll have someone to lean on when you tire (Lesson 9). California aligned policymakers, payers and providers—a powerful, successful model.
  2. Start by looking inward. The road to equity begins at home. Organizations should examine their own tendencies toward bias, such as in hiring. Organizations whose internal demographics and operations embody equity are well positioned to do the same externally.
  1. Smash silos. Looking inward includes integrating disciplines and departments. In response to NCQA’s mantra—there is no quality without equity—one speaker quipped, “There is no quality without IT!” Indeed, improving quality and equity involves IT, HR, operations—and myriad other parts of the organization.
  2. Keep interventions flexible. A “one-size-fits-all” approach (what one speaker dubbed “a size medium t-shirt strategy”) can alienate the people you want to help. Understanding what a community needs helps avoid unintended consequences. Adapt your approach! Develop interventions that address the needs and circumstances of the communities you serve.
  3. Tailor your terminology. The need for flexibility extends to what you call your work. The term “health equity” might not resonate everywhere. Find different starting points to engage different communities. This lesson highlights the importance of culturally competent communication plans. Use words that work.
  4. Secure, granular data is your superpower. Data equity—collecting, analyzing and interpreting data to promote fair and inclusive decision making—is a cornerstone of health equity. Collecting detailed data helps you “see” people as complex individuals. Be ready to explain (especially to vulnerable and marginalized groups) how you will protect and use what they tell you about their race, ethnicity, language, sexual orientation and gender identity. And respect people’s decision to opt out of giving you that information. That’s part of being flexible and “meeting people where they are.”
  5. Get ready to play the long game. It took decades, even centuries, for current inequities to calcify. It will take years to fix them. Settle in for steady, sustained effort. When—not if—momentum stalls or you feel discouraged, get “gritty.” Accept that progress won’t be linear. Celebrate small, incremental wins, and draw strength from colleagues who share your commitment to better, more equitable care. (Again, Lesson 9!)
  6. Connect with clinicians’ original idealism. To secure clinician buy-in to what might be an awkward examination and reset of long-standing practices, help them “get out of their heads and into their hearts.” Most doctors and nurses entered medicine to help people. Turn them into equity advocates by helping them contrast that idealism with reality: The current system often fails people who need them most.
  1. Inspire and elevate each other. All lessons culminate in the Health Equity Forum’s overarching, animating purpose. Speakers exhorted participants to find inspiration in each other, and to focus on shared goals. Embrace the hope and optimism that working for health equity means you’ve joined a “Justice League” to end unfairness and build a better, healthier society.

New Findings, New Guidance

The refreshed information includes:

  • Health plans we identified as responsible stewards of antibiotic use from 2020 to 2022—plus lessons from those plans’ success.
  • Insight into trends of antibiotic use, beginning in measurement year 2020.
  • New resources highlighting emerging research on antibiotic stewardship.

And our education programs offer free continuing education credits.

The Big Picture

Inappropriate antibiotic use:

  • Is a long-standing public health problem. That’s why we write about it often.
  • Can expose patients to medication side-effects and lead to antibiotic resistance—where drugs designed to kill germs like bacteria no longer work.
  • Is more common than you might think. The CDC reports that about 35,000 people die each year from antibiotic-resistant infections. To give some perspective, that’s roughly in the middle between the number of people who die from homicide (23,000) and suicide (48,000).

Health plans:

  • Play an important role in promoting appropriate antibiotic use.
  • Can develop programs to support better prescribing practices.
  • Can educate members about illnesses that call for an antibiotic—or don’t.

Quality measures help health plans in those efforts.

HEDIS includes antibiotic prescribing measures for three illnesses that are the main cause of inappropriate prescribing:

  • Bronchitis/bronchiolitis.
  • Pharyngitis (sore throat).
  • Upper respiratory infection.

The measures help plans identify, highlight and monitor appropriate prescribing.

The Trouble With Today’s Health Data

Remember when health data were scarce? Those days are over.

Today, the amount and sources of data are vast. Clinical, administrative, consumer, patient-reported data (including from wearables) flow freely.

That’s the question examined by the “Making Data Useful” track at the NCQA Health Innovation Summit, October 23–25 in Orlando.

  • Mark A. Clements, professor of pediatrics and pediatric endocrinologist at Children’s Mercy Kansas City, an expert on how big and small data can improve type 1 diabetes care.
