NCQA is at the forefront of these efforts, with rigorous programs to make sure that delivery-system reforms protect consumers and deliver results.<\/p>\n<p>NCQA recognizes practices that follow the medical home model and transform their operations to meet patients needs. PCMHs provide:<\/p>\n<ul>\n<li>Enhanced access to care through expanded hours, online communications and long-term partnerships with clinicians, rather than sporadic, hurried visits.<\/li>\n<li>Highly coordinated care \u2014 especially for preventive services and those with chronic conditions \u2014 that includes connections to other clinicians and community supports.<\/li>\n<li>Shared decision-making between providers and patients.<\/li>\n<\/ul>\n<p>Our Patient-Centered Medical Home (PCMH) recognition program plays a key role in primary care reform initiatives across the country.<\/p>\n<h2>PCMH &amp; The States<\/h2>\n<p>Twenty-five public sector medical home initiatives across 22 states require or incentivize NCQA PCMH Recognition. More information\u00a0<a href=\"https:\/\/www.ncqa.org\/programs\/health-care-providers-practices\/patient-centered-medical-home-pcmh\/benefits-support\/payer-support\/directory\/?provider-org-type%5B%5D=state-initiative&amp;provider-state=\">here<\/a>.<\/p>\n<ul>\n<li>What\u2019s the value to states?\n<ul>\n<li>Provides\u00a0a\u00a0standardized model of care.<\/li>\n<li>Improves\u00a0outcomes, lowers utilization.<\/li>\n<li>Aligns\u00a0with MACRA\/MIPS and CPC+.<\/li>\n<li>Builds\u00a0provider capacity,\u00a0improves\u00a0access.<\/li>\n<li>Supports\u00a0value-based payment initiatives.<\/li>\n<li>Enhances\u00a0state quality improvement efforts.<\/li>\n<li>Foundation\u00a0for building\u00a0accountable care organizations\/clinical integration\u00a0networks.<\/li>\n<li>Drives\u00a0savings.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><em><strong>State Spotlight:\u00a0Tennessee.<\/strong><\/em>\u00a0Tennessee uses NCQA PCMH Recognition to improve care delivery for their Medicaid patients. That\u2019s where the \u201cmedical home neighborhood\u201d comes in. NCQA has products that bring the medical home model to all points of care: specialists, urgent care clinics, case managers and more<\/p>\n<p>NCQA\u2019s <a href=\"https:\/\/www.ncqa.org\/programs\/health-care-providers-practices\/patient-centered-specialty-practice-recognition-pcsp\/\">Patient-Centered Specialty Practice (PCSP)<\/a> recognition program addresses gaps in quality that occur due to a lack of coordination between primary care and specialty practices.<\/p>\n<p>NCQA\u2019s <a href=\"https:\/\/www.ncqa.org\/programs\/health-care-providers-practices\/patient-centered-connected-care-pccc\/\">Patient-Centered Connected Care<\/a> program for urgent care sites ensures a relationship between clinics and primary care doctors.<\/p>\n<p>NCQA has developed\u00a0a <a href=\"https:\/\/www.ncqa.org\/programs\/health-care-providers-practices\/school-based-medical-home-recognition\/\">School-Based Medical Home<\/a> program that recognizes high-quality school health centers that provide comprehensive medical care to children and adolescents in their districts.<\/p>\n<p>Learn more about the PCMH and other medical neighborhood programs <a href=\"https:\/\/www.ncqa.org\/programs\/health-care-providers-practices\/patient-centered-medical-home-pcmh\/\">here<\/a>.<\/p>\n<p>&nbsp;<\/p>\n<h2>PCMH Evidence<\/h2>\n<p>Patient-centered medical homes are driving some of the most important reforms in health care delivery today.
