This page allows organizations with CMS Innovation Center model participation agreements to download the applicable measure specifications and related content for use in specific models.<\/p>\n<p>If you have any questions related to the CMS Innovation Center models, please direct them to the <a href=\"https:\/\/www.cms.gov\/priorities\/innovation\/models#views=models\">CMS Innovation Center<\/a>.<\/p>\n<p>If you have a question about a NCQA measure used in a model, please submit the question to NCQA\u2019s <a href=\"https:\/\/my.ncqa.org\/Esign\">Policy Clarification Support (PCS) System<\/a>. Choose \u201cCMS Innovation Center Models\u201d for product\/program type. Next, please select the model you have a question about for general content area. Choose the measure you have a question about using the specific area dropdown menu. Lastly, please give a detailed description of your question, and the NCQA PCS Team will get back with you.<\/p>\n<h2>NCQA Measure Specifications for CMS Innovation Center Models<\/h2>\n<p>The table below provides a list of all CMS Innovation Center models in which NCQA measures are currently used. To access measure specifications and related content for a specific model, click on the applicable model under the \u201cModel Specifications\u201d column. A window will pop up with a form to complete before access is provided.
