They currently have eight PCMH-Recognized practices\u2014Champlain Islands, Essex, Good Health, Pearl Street, Riverside, Safe Harbor, South End and Winooski\u2014serving more than 28,000 unique patients each year. If you anticipate submission delays, please notify your assigned Recognition Programs Representative in advance.
