Tables Usage Agreement<\/h2>\n

Use of these tables constitutes acceptance of our terms of use as set forth in this usage agreement (the \u201cAgreement\u201d).<\/p>\n

You may not use the Risk Adjustment Tables for any other purpose, including, but not limited to, a commercial or research purpose, unless you obtain prior written approval from NCQA and agree to the terms of a license, at NCQA\u2019s sole discretion.<\/p>\n

You may not copy electronically or otherwise transmit, redistribute, republish, modify, adapt, translate, reverse engineer, decompile, disassemble, sublicense or otherwise provide the Risk Adjustment Tables to any third party. You may not use or include the Risk Adjustment Tables in any other print or electronic publication service or product.<\/p>\n

This Agreement shall become effective immediately upon your access of the Risk Adjustment Tables.<\/p>\n

YOU CONFIRM YOUR AGREEMENT TO VOLUNTARILY USE THE RISK ADJUSTMENT TABLES SUBJECT TO THE TERMS AND CONDITIONS OF THIS AGREEMENT BY USING THE RISK ADJUSTMENT TABLES FOR HEDIS MEASURE CALCULATION.<\/p>\n

IF YOU DO NOT AGREE TO THE TERMS STATED ABOVE YOU ARE PROHIBITED FROM USING ANY OR ALL OF THE NCQA HEDIS RISK ADJUSTMENT TABLES.<\/p>\n
